The primary outcomes to be assessed include early and exclusive breastfeeding and the adoption of modern contraception at 1 year postpartum.

To measure the impact of Kilkari, surveys will be administered at baseline and endline to collect data on phone ownership, geographical and demographic characteristics, knowledge, practices, respectful maternity care, and coverage for antenatal care, delivery, and postnatal care. In-depth interviews and focus group discussions will also be conducted to understand user perceptions of Kilkari and experiences providing phone numbers and personal health information to healthcare providers.

The study will also assess the economic costs of Kilkari by measuring program, user, and health system costs. The Lives Saved Tool will be used to estimate the number of lives saved and the incremental disability-adjusted life years (DALYs) averted associated with Kilkari. The cost per DALY averted will be calculated to determine the cost-effectiveness of the program.

Statistical analyses will be performed using intention-to-treat methodology, adjusting for baseline factors. Regression models and hypothesis tests will be used to analyze the primary and secondary outcomes. Subgroup analyses will be conducted to explore heterogeneity in effects.

Overall, the study aims to provide robust and comprehensive evidence on the effectiveness and cost-effectiveness of Kilkari in improving maternal and child health outcomes in India.
